: Basal cell carcinoma (BCC) is a slow growing, locally destructive, malignant tumor of the skin. It derived from non- keratinizing cells that originate basal layer epidermis. After excision nasolabial flap, median forehead dorsal nasal glabellar flaps, bilobed cheek and craniofacial flaps are used for reconstruction. The Zitelli’s flap one most useful simple double transposition designed to move more skin, without deformation. A 60-years old male patient reported our department with complains circular raised reddish discoloration lesion on left side nose. Surgical reconstruction defect using was planned under general anaesthesia. in such manner could be turned over ala nose closure donor site done primarily deformity. performed flap. has been followed up 1year no evidence recurrence. versatile reliable coverage small skin soft tissue defects lower third gives successful outcome if it well properly.
